PERSI Choice

This form is used by employees to manage payroll contributions to the PERSI Choice 401(k) Plan (Plan ID: 95270-01).

Please Note:

• PERSI (Public Employee Retirement System of Idaho) is not optional; participation is required by state law for all eligible employees.
• The PERSI Choice 401(k) is an optional supplemental retirement savings plan that allows employees to contribute additional pre-tax income toward retirement.

Purpose of the Form

This form allows employees to:

• Start, restart, change, or stop payroll deductions for the PERSI Choice 401(k) Plan.
• Elect additional Age 50 Catch-Up Contributions, if eligible.

Important:
For changes to take effect, please submit the completed form to the District Office by the 10th of the month.
